At the Des Plaines Community Foundation, we are committed to empowering mobility and independence for low-income residents with disabilities through our Wheelchair Ramp Loan Program. This initiative provides temporary wheelchair ramps to individuals and families in need, ensuring safe and easy access to homes. By removing barriers and fostering inclusivity, we aim to enhance the quality of life for our community members. HANDICAPPED ACCESSIBLE RAMP AGREEMENT

For as Long as They Need the Ramp

I agree to pay a refundable deposit of $1.00 to DPCF for the use of said ramp.

I agree to notify and return the handicapped accessible ramp to DPCF when I no longer need the ramp.

DPCF will refund the deposit when the handicapped accessible ramp is either returned to DPCF or is picked up by DPCF. Should the handicapped accessible ramp not be returned, the deposit shall be forfeited to DPCF. By submitting this form, you agree to these terms.
